The project's objective was to present a model for the church to lead efforts to reduce health disparities in underserved communities through collaborations with government and private organizations. Qualitative research was conducted to assess needs and measure project effectiveness, using surveys and interviews. This effort increased health education, outreach, awareness, and identified support resources within the community. The methodology used was the Proactive Research Method which pro-actively worked toward a transformation of their behavior as noted thorugh qualitative data. It was found that by utilizing several techniques addressed in this project their quality of life was improved as a result of 'spiritual' education and positive 'spiritual' practices.
