This qualitative action research project document describes the actual establishment of a congregational-based CPE program in Fargo, North Dakota.  Using this information and data drawn from the experience of five other founding supervisors of congregational-based programs, fourteen developmental steps and seven founding principles were found to be necessary in the development of a viable congregational-based CPE center. Methods used included discernment, questionnaires, interviews, interactive programs, and walk-arounds.  In this exploratory case study, the church participants learned who they are, who their neighbors are, and what they are called to do, through spiritual, contextual, historical, organizational, missional, attitudinal, relational, educational, and connectional components. The project used a set of interfaith activities and was implemented in Charleston, West Virginia in a series of meetings between members of a local church and the Islamic center. The results were positive, as participants characterized the activities as a valuable learning experience that changed their perceptions of each other's religions. The project involved two groups of participants that were studied and evaluated. The diagnosis included dementia and Alzheimer's disease (Group A), and depression or serious medical or neurological illnesses (Group B). Using the participant/observer approach, all participants kept daily journals, answered a questionnaire, and weekly meetings were taped. This project found individuals feeling closer to God, gender response differences, and occasionally, typical Appalachian responses. In this project, eighteen clergy from different denominations in the Greater Kanawha Valley in West Virginia participated in focus groups addressing questions selected by the moderator. Clergy members were encouraged to talk freely about their concerns and perspectives. Verbatim transcripts were generated from three focus-group discussions.  Quotes from clergy participants were grouped into thematic areas for the purpose of qualitative analysis.
