- The context for this research project is Bethel African Methodist Episcopal Church in Georgetown, South Carolina. The project's objective was to present a model for the church to lead efforts to reduce health disparities in underserved communities through collaborations with government and private organizations. Qualitative research was conducted to assess needs and measure project effectiveness, using surveys and interviews. This effort increased health education, outreach, awareness, and identified support resources within the community. This model may also increase access to healthcare, reduce chronic diseases, eliminate barriers to care and improve the quality of life for persons living in underserved communities.
